- Income and source of income (recent pay stubs, prior year's tax return, disability or unemployment verification)
- Tax returns documenting income for year first billed or 12 months prior
- Pay stubs from within a 6-month period before or after first billed (or when application submitted for pre-service)
- For Discount Payment: proof of medical expenses from other facilities (bank/credit card statements, medical bills, insurance EOBs)
- Medi-Cal denial letters/statements if available
