• Signed attestation under penalty of perjury if income documentation unavailable (at Rady Children's sole discretion)
• Documentation of High Medical Costs including receipts/invoices of expenses paid (signed attestation NOT accepted for High Medical Costs)
• Documentation of presence or absence of Third-Party Coverage
