What you'll need to apply
- Income tax returns OR recent pay stubs (documentation limited to these for Charity/Discounted Care eligibility)
- Bank/checking account statements evidencing available resources (per base FAP)
- Documentation of Essential Living Expenses (only if requesting an extended payment plan)
- Documentation of assets may be accepted but shall NOT be required
